Requirements
- Must be 18 years of age
- BLS and First Aid
- Authorized to work lawfully in the United States.
- Negative TB skin test of chest Xray
- Must be able to pass criminal background Check
- Must have reliable transportation; valid driver’s license and proof of insurance is required
- Ability to work with limited supervision
- Able to lift up to 50lbs, stand, walk, squat, kneel, bend, reach, twist, push and pull. Varied assistance will be required depending on mobility of the client.
- Ability to follow written and verbal instructions
- Good communication and interpersonal skills
CNA/ PCA Position Competencies/ Essential Duties:
- Must be CNA licensed by the state of Georgia.
- BLS and First Aid
- Nurturing and compassionate nature with the desire to take care of others
- Ability to work with limited supervision
- Able to lift up to 50lbs, stand, walk, squat, kneel, bend, reach, twist, push and pull. Varied assistance will be required depending on mobility of the client.
- Ability to follow written and verbal instructions
- Comfortable using an electronic device
- Follow specific care plans for clients and report on completed tasks.
- Assist with personal care needs of the client (bathing, dressing etc. ).
- Provide or assist in routine house cleaning, meal preparation, and laundry.
- Assist client with the self-administration of medications.
- Observe and report any changes in client’s condition.
- Maintain a high degree of confidentiality at all times due to access of sensitive information.
- Maintain a regular, predictable, consistent attendance and is flexible to meet the needs of the department.
- Follow required HIPAA regulations and requirements.
- Abide by all rules and regulations, policies, procedures and standard.

LPN Position Competencies/ Essential Duties:
- Must have Current LPN license
- CPR Certification
- Clinical documentation experience
- Implement treatment and plan of care, using sound judgment and critical thinking
- Maintain accurate, detailed, and timely documentation consistent with the plan of care and the nursing processes
- Perform all skill, treatments, and procedures to the scope of practice as defined in the Nurse Practice Act in the state of clinical practice and adhere to the policies, procedures, and protocols of the agency of employment
- Must have Current RN license
- Experience with IVs, PICC lines/Ports
- CPR Certification
- Responsible for starting client’s which includes a comprehensive nursing assessment, medication reconciliation, safety assessments and participate in telehealth rounds with physicians,
- Monitor patient’s condition and assess their needs to provide the best possible outcome.
- Observe and interpret patient’s symptoms and communicate them to physicians.
- Collaborate with physicians and nurses to devise individualized care plans for patients.
- Perform routine procedures (blood pressure measurements, administrating injections etc.)
- Adjust and administer patient’s medication (typically intravenously) and provide treatments according to the physician’s orders.
- Provide instant medical care in emergencies.
- Educate the patient and family members on how to manage the illness with the goal of recovering the health and well-being of the patient.
